DFWHC Membership Directory

This directory is produced on an annual basis and includes up-to-date listings of DFWHC's Hospital, Associate and Personal members. Staff listings, addresses and phone numbers for each member are also included. The Membership Directory is initially available for free to all DFWHC members. Extra copies are $15 each. Consumer Map Brochure
